Parts A / B - 2014 Printing Parts A & B need to be complete by participants in all Scouting events. These 2 parts alone will work for short term (weekend) events lasting less than 72 hours. This is the same Part A & B that is included with the full Annual Health and Medial Record Form, so if you have completed parts A & B previously for summer camp, they are valid for 1 year from date of signature. Parts A / B / C - 2014 Printing This PDF contains Parts A, B, with the addition of part "C" which is a physical exam, certified by a licensed heath-care provider (MD or DO) for events that are longer than 72 hours or the activities are strenuous and demanding. This is the form for Summer Camp and High Adventure and it can be used for short term camping as well. It is valid for only 1 year from date of signature. Colorado Medical Form Addendum Yes, unfortunately this is still required for Scouts (not parents or siblings) in Colorado for Day Camps, Peaceful Valley (Camp Dobbins and Camp Dietler), Tahosa (EaglePoint, Tahosa Trek, Alpine Adventure, and Colorado Adventure Weeks), Big Horn, Wood Badge or any event over 72 hours. Over-The-Counter Medication Authorization To insure proper care is given to Scouts for even minor discomforts, the Troop as adapted an Over-The-Counter medication authorization form used by high adventure BSA camps to accompany the BSA standard medical form. This is not required, by highly advisable for your Scout's comfort. Without specific authorization, these medications cannot be administered.
